I wondered if it was carelessness, or if she had slipped the child a small dose to quiet him up.

Back in the day, they used to have a product called Bayers Soothing Syrup, which was intended for Colic but was widely used otherwise. Its only active ingredient was heroin. Very effective colic treatment, but the dose has to be managed like any drug.

Methadone is far more potent than heroin in terms of amounts needed to put someone in respiratory arrest. Something like 10 fold more potent.
